Meet Dr. Schwartz
Gary D. Schwartz is board certified in internal medicine and has been in private practice for twenty years. Dr. Schwartz completed his internal medicine residency at The University of Medicine and Dentistry of New Jersey (now Rutgers New Jersey Medical School) at Hackensack University Medical Center. As a medical student, Dr. Schwartz conducted his clinical training in London, England.
Dr. Schwartz has been involved in healthcare since the age of 18. He worked on an ambulance as an EMT in Brooklyn. He became a Registered Nurse (RN) and worked in the emergency room before further pursuing his medical degree. Dr. Schwartz is a Senior Attending in the Department of Internal Medicine at Hackensack University Medical Center and an Attending at Holy Name Medical Center. Dr. Schwartz has a passion for primary care, preventative medicine, and is known for being a premier diagnostician.

Concerned about numerous healthcare trends that are negatively affecting doctor-patient relationships and the way care is delivered, Dr. Schwartz has collaborated with Castle Connolly Private Health Partners (CCPHP) to create a unique, concierge or membership-based practice model that provides a more optimized healthcare experience for both patients and providers.
In this model, Dr. Schwartz has considerably fewer patients than typical primary care doctors. This allows him to have enough time to provide each and every patient with personalized, unhurried care. Members of Schwartz CCPHP pay an affordable annual fee to take advantage of a wide array of member benefits and enjoy a more convenient, connected, and compassionate approach to healthcare.
Membership fees are payable to Schwartz CCPHP, LLC, a business entity formed by Dr. Schwartz and CCPHP that is separate from his medical practice. Dr. Schwartz provides all professional services through his practice, Schwartz CCPHP. Since the membership fees do not pay for any insurance-covered healthcare services, we recommend that our members maintain their health insurance. Contact us for more information.
Practice Philosophy
As a physician, my greatest satisfaction is derived from working closely and comprehensively with my patients in an unhurried environment, so that I can be my patients’ advocate and advisor regarding longevity and quality of life.
In order to alleviate some of the barriers in today’s healthcare environment that interfere with my ability to continue to practice this way, I have collaborated with Castle Connolly Private Health Partners, LLC (CCPHP), to create a new concierge program called Schwartz CCPHP. The program provides my patients with a wide variety of membership amenities to enhance their care experience, such as access to the innovative SENS Solution® Wellness Program, which includes a personal health coach who collaborates with me to optimize their wellbeing.
The patient-physician relationship is what I value the most as a practitioner. With this model, I will be able to provide a connected, convenient, collaborative care approach on a more ongoing and consistent basis. Ultimately, this program allows me to get back to putting you, my patient, first.
